Two forms are required to purchase an individual mail box. Please print the following forms, complete, sign and return. If you sign this form in our presence no notarization is required. If you are off island you will need to have your signature notarized.
Requirements listed in # 8 on postal for is also required.
Individual Mail Boxes are $15.00 monthly - Deposit of $15.00 is also required. |
